Good question. Since Star Wars as general constantly ignores the "physics" I don't think it is necessary to question the procedure.

I wouldn't at all be surprised if the technology exists in Star Wars to do that.

I think it is theoretically possible, but it is far more convenient to get a cybernetic one, given the advantages it offers.

I'm not sure at what point it became possible, but the Young Jedi Knights series I believe confirms that had Tenel Ka's lightsaber not exploded in her severed arm after Jacen accidently cut it off they would've been able to reattach it.
