Asajj Ventress vs Yoda (TPM)

If this is a mismatch for Yoda, then here's round 2: Ventress vs TPM Yoda & TPM Mace.

Ventress gets slaughtered both rounds.

Logically, Yoda still smacks the crap out of her. Not seeing much of a reason why he'd have grown immensely more powerful between TPM and AOTC (a few months after which he was capable of ragdolling and stomping her with the Force), since he wasn't actively involved in any conflicts or going through any significant emotional growth of note.

Ok lol. 

In a team battle, AOTC Yoda and AOTC Anakin > Any version of Ventress. If she fought AOTC Anakin one on one then sure, she'd probably win but there's no way through stars she's winning against Yoda.

Frankly, this is a mismatch. The best Ventress can realistically hope for is to contend and escape with her life and pride intact. Despite his shortcomings, Yoda simply outclasses her on pretty much every conceivable level. She may be able to contend with top-tier council masters as a duelist, but against the very best of the Jedi, that's about all she can hope for. She simply really isn't bringing anything to the table that Yoda hasn't already been shown dealing with; except perhaps her little trick of reanimating the dead, but that is extremely situational and likely a very intensive power that isn't going to be just flicked on in the midst of a heated duel. I reckon there is a fair chance of this ending without Yoda needing to draw his blade, but should he fail to penetrate her TK defences, or she somehow blindside him and close the distance, a lightsaber duel between these two would be over in moments. I simply don't see Ventress defending against Yoda's onslaught for long, skilled as she is.
